California State form DE 48 is required. The SSN or Corporate Identification Number must be completed at the top of the form as well as the SSN of the person signing at the bottom or it will be rejected. If a current Form DE1 is on file with the state, the signer of the POA must be a Corporate Officer listed in section R of the current Form DE1 A copy of Form DE1 on file with the state must accompany the POA. If the client has not completed a Form DE1, the client must complete one and submit with the POA. The person signing the POA must be listed as a corporate officer in section R of the Form DE1

North Carolina state Form NCUI 604 must accompany ADP Form 6018. This is only required if the client does not already have a Form NCUI 604 on file with the agency with a matching signature.
